A Second Touch Support Coordination Agency is seeking experienced support coordinators for adults 21 and over with intellectual disabilities. A Second Touch Services Atlantic, Camden, Cumberland, Gloucester, and Burlington County.

Work remotely from home and in the Community.

(MANDATORY OFFICE HOURS ON THURSDAYS 10 AM-12 PM)

WE ARE HIRING FOR CAMDEN and GLOUCESTER COUNTY ONLY.

Responsibilities 

Initial in-home intake services that assist participants in gaining access to needed programs and State plan services, as well as needed medical, social, educational, and other services.

Quarterly face-to-face monitoring and weekly phone calls are required. The Support Coordinator is responsible for attending the individualized service plan team meetings, and maintaining the individualized service plan with the participant, their family, and other team members designated by the participant as needed and annually.

The candidate will be hired with a caseload of 10 to 15 participants. Support Coordinators will not be required to write plans.

Starting Rate: $72 per client. After 90 days, rate increases $77 per client. 3% company match retirement plan Simplified Employee Pension IRA (SEP IRA). Mileage Reimbursement, Company laptop and work phone provided.

Candidate must be experienced in Irecord; understanding of DDD supports program and CCP. A family-oriented, understanding individual and a fast learner who can easily learn specific writing techniques as well as extremely organized, compassionate, time-sensitive, and able to work with the supervisor as well as independently from home. Experience with understanding developmentally disabled individuals and organizational skills is a condition of hire.

Experienced in Support Coordination and Irecord system Preferred. If required: A two-day mandated training course with the State of New Jersey. The candidate will be paid $60 per day totaling $120. Training will be paid after working for 90 days.

Qualifications Direct Support Professional:

  • Bachelor's Degree or higher
  • 1 year of experience in Developmental Disabilities or Care Management/Coordination in any social services field
  • Successfully completed training is required by the Division of Developmental Disabilities before rendering any service.
  • Ability to pass mandatory drug screen and physical.
  • Pass a criminal history and personal background checks.
  • Requires basic language, written and computer skills necessary for communication and Documentation.

A Second Touch support coordination agency is now hiring Part-time experienced Direct Support experienced professionals for adults age 21 and over with intellectual disabilities. A Second Touch service DSP in the following Counties: Bergen, Cape May, Ocean, Essex, Hudson, Hunterdon, Mercer, Middlesex, Monmouth, Morris, Passaic, Salem, Somerset, Sussex, Union, and Warren Counties.

Professionals will administer medication in accordance with medical provider directions and DSDC /State of NJ Policies. Gains and maintains knowledge of health and medical issues for all assigned individual(s) supported. Assists individual(s) supported with healthcare visits, medications, and therapy plans. Notifies the appropriate supervisor of any health concerns or needs for the individual(s) supported.

Follows all policies and procedures regarding safety in all environments.

Active Treatment

Implements individual support plans as written. Aids and supports individual(s) in activities of daily living. Teaches appropriate skills and involves individual(s) supported in meaningful and valued activities. Provides and seeks out opportunities for individual(s) supported to make choices. Obtains knowledge of individual(s) supported desired outcomes and works to ensure they are integrated into daily activities. Incorporates time, space, and opportunity for privacy in the life of the individual(s) supported. Maintains strict confidentiality of all personal information concerning individual(s) supported. Knows, advocates, and respects the rights of individual(s) supported.

Treats all individual(s) supported, families, coworkers and supervisors with dignity and respect.

Community involvement

Demonstrates the importance of community involvement, social interactions, and maintaining relationships. Develops and facilitates community connections and relationships. Assists individual(s) supported in developing and following a person-centered schedule. Aids individual(s) supported to participate in activities of their choosing.

Accountability:

Handles and maintains individual/agency finances ethically, honestly, and accurately with appropriate documentation and receipts. Follow all agency policies and procedures. Provides emergency coverage when requested by management. Takes initiative in performing additional tasks as needed or as assigned.

Provides clear, accurate and timely records and documentation.

Professional Conduct

Provides positive and appropriate feedback to coworkers, team members, supervisors, and agency representatives. Participates in and completes periodic training and certification as required. Maintains confidentiality of records/information according to HIPAA, State, Federal laws, and Guidelines. Reports all instances of abuse, neglect, mistreatment, and exploitation immediately to the appropriate supervisor.

Displays conduct in the community that enhances the image of individual(s) supported and the agency.

Qualifications Direct Support Professional:

  • Candidate must be at least 19 years of age and have a valid driver's license for at least 3 years, acceptable driving history and proof of auto insurance.
    High school diploma or equivalent.

    Ability to pass mandatory drug screens and physicals.

    Pass a criminal history and personal background checks.

    Ability and willingness to assist individual in activities of daily living including bathing, toileting, and other self-help skills

    Ability and willingness to support individuals with challenging behavior.
